Insurance Information and Financial Policy

Some insurance companies require pre-authorization prior to obtaining specialty care. Please contact your insurer directly to determine if you need a referral or pre-authorization. If you have questions, you may call our Business Office at 327-3370 for assistance.

Please bring your insurance card and co-pay amount with you to your appointment.

You may pay out-of-pocket costs by check, cash or credit card.

We do offer payment arrangements through our Business Office, including automatic payments via credit card and payment plans.

The parent authorizing treatment for a child will be responsible for charges related to care. If a divorce decree requires the other parent to pay all or part of the treatment costs, it is the authorizing parents’ responsibility to collect from the other parent.

Our Business Office personnel are well versed in negotiating the maze of insurance payments, co-pays and deductible. Please feel free to utilize their services.
